Personal information is collected directly from clients or potential future clients when they personally complete and submit a client card and consent form to become a client of Cosmetik Aesthetik. This can be done in person or via our website portals or links. Personal contact information may also be received via an online enquiries form.

Personal Information is information or an opinion that identifies an individual. Examples of Personal Information we collect include names, addresses, email addresses, phone and facsimile numbers.

This Personal Information is obtained in many ways including correspondence, by telephone, by email, via our website www.cosmetikaesthetik.com.au, from your website, from medical forms, as required by law, from media and publications, from other publicly available sources, from cookies and third-parties.

You are not obligated to provide personal information however failure to do so may result in Cosmetik Aesthetik being unable to provide services or products to you.

• To fulfil obligations pursuant to a client’s request for, and Cosmetik Aesthetik agreement to provide, services, products and/or any other agreement between him/her;
• To render services to the client;
• To obtain opinions or comments about products and/or services from members;
• To record statistical data for marketing analysis from clients.
• To provide information about products, services and/or special offers;

For the purposes of this Privacy Policy, sensitive information means the information a client has disclosed to us about their physical and mental health and well-being.

Sensitive information will only ever be used for providing services to clients. Such information will not be used or disclosed for any other purposes without consent, except in cases when disclosure may be required by law or is necessary to protect the rights or property of Cosmetik Aesthetik, or to lessen a serious threat to a person’s health or safety. For the purposes of this Privacy Policy sensitive information is a subset of personal information.

We do not disclose, sell or trade any personally identifiable information to any external party. Non-identifiable information may be released to help us with marketing and other information needing to be collated. This excludes our affiliated third parties who assist us in operating and conducting our business, as well as offer third party products and services on our website. Those companies will be permitted to obtain only the information they need to deliver the service and adhere to their own Privacy Policy.

Accessing your Personal Information:

You have the right to review personal and/or sensitive Information that may be recorded on Cosmetik Aesthetik database. Information may be reviewed by submitting a written request for such access.

A request may be made to delete or amend personal and/or sensitive information, and all reasonable steps to delete the information will be made, except where it is required for legal reasons, which includes but is not limited to the proper maintenance of medical and health records. Deletion or amendment of information may result in us being unable to sell products directly and/or provide certain services including after sales services.

We do not send advertising information without obtaining prior consent. If an individual receives communications from us which they do not wish to receive, they may remove their name from the marketing database by contacting us via the enquiries form on the website.

Marketing:

Cosmetik Aesthetik uses the communication platform MailChimp, this service is used for delivery of email updates, promotions, clinic news and newsletters only. Your personal information (name and email address) is used for delivering these communications. Please refer to Mailchimp’s privacy policy for further information.

All personal data entered on www.cosmetikaesthetik.com.au is kept strictly confidential with the managing director.
